Acute Inferior MI with RV infarct ST elevation in II, III, aVF. Reciprocal ST depression in I, aVL, V1-V3 (V2-V6 in this case). See ST T abnormalities in acute ST elevation MI table V3R V4R
Note ST elevation > 1mm. in V3R and V4R Right side chest lead. Right side chest lead should be obtained at the same time as initial ECG or within 6 hours after the onset of MI to assist the diagnosis of RV infarct associate with acute inferior MI.
