RBBB, typical
Patient. 50 year old male was admitted for post operative bleeding from recent tonsilectomy. He has history of hyperlipidemia and family history for heart disease. Patient has a negative stress radionuclide imaging study 7 months ago.

Criteria for typical RBBB
  1. QRS duration is > 120 msec.
  2. Broad, notched R wave (rsR', RsR', rSR', rSr', rsr') in right precordial leads V1 and V2.
  3. Wide S wave in lateral leads V5, V6 (with leads placement far lateral enough), lead I.
  4. R wave (in lead I and V6) should be narrow (normal). If it is wide, there is other cause such as intraventricular conduction defect.
