Posterior MI (True posterior MI or Posterolateral MI)
Patient. 61 year old male had history of PCI 3 years earlier. The detail was not available. He was admitted this time with chest pain and abnormal troponin. Coronary angiographic study revealed near occluded mid circumflex lesion which received PCI and stent placement.
Comment. The prominent R in V1 (R/S >1) in this case may be due to true posterior or posterolateral MI, normal variant or right conduction abnormality.

Posterior MI (True posterior MI or Posterolateral MI) with inferior MI
Patient, 74 year old male suffered an inferoposterior MI. He underwent PCI and stent placement to the complete occluded circumflex artery. Echocardiogram revealed hypokinesis of the LV posterolateral wall.
Comment. Posterolateral MI is commonly associate with inferior MI. Prominent R in V1 in this case fovor the diagnosis of posterior MI than other diagnoses menttioned above.
